Service Coordinator

SEM Villa provides an on-site SERVICE COORDINATOR.  His name is Jeff Seip. He is available Monday thru Friday, 9:00AM until 5:00PM. Jeff can be reached at (513) 831-3720.  Please take a moment to review the many ways Jeff can be of assistance:

FINANCIAL ASSISTANCE

• Assistance with medical billing questions
• Enrolling in benefit programs
• Provide information/enroll in entitlement programs
• Assist with securing a payee/guardian

MEDICAL ASSISTANCE

• Assistance with prescription drug benefit plans
• Arrange for home health aide services
• Coordinate wellness clinics
• Assistance with Medicare/Medicaid bills
• Arrange for Visiting Physicians Appointments
• Coordinate with hospital discharge planners
• Assist with setting up emergency medical information
• Assist with pharmacy options / transfers of meds
• Provide information about “LIFELINE” services

EDUCATION AND EMOTIONAL SUPPORT

• Provide support and encouragement services
• Arrange for wellness seminars on elder topics
• Arrange for Library resources to be delivered
• Make referrals to Grief/Loss Counselors/Groups
• Provide requested conflict resolution services

HOMEMAKER ASSISTANCE

• Assist you in finding and arranging for homemaker services
• Arrange for medication-reminders
• Arrange for transportation services as needed
• Provide information about “walkers” as needed
• Assist with obtaining needed safety items for bath/room
• Assist you in arranging for personal care services

BUSINESS ISSUES

• Assist in completing forms or obtaining information regarding living wills or directives
• Assist with telephone discount program application
